Dear
New Patient:
Welcome
to Florida Pain Management. Please take a moment to familiarize
yourself with our practice guidelines. Prior to being seen for
a consultation at our office, you will need to print out and
complete the new patient information packet. Please include
all relevant medical history and information, as this will be
used to help us create your unique treatment plan. If your completed
forms are not brought with you, your appointment may be re-scheduled.
It
is also very important for you to bring the report and films
of your most recent MRI, CT scan and X-rays with you to the
consultation. If you are currently on pain medications, please
bring in the bottle(s) and list of the medications you are taking.
We also need to have any old records that are related to the
previous treatment of your pain.
Please
be aware that your first visit to our clinic is only an evaluation
and that narcotic pain medication may not be prescribed.
The
physicians at Florida Pain Management believe in comprehensive
pain management, which may include any of the following:
• referral to physical therapy
• interventional pain management
• mental health evaluation by a psychologist
• referral for massage therapy
• pain medication management
• random urine drug testing
To
avoid any charges for missed appointments, kindly give us a
24-hour notice of cancellation.
